Athlete Registration

Welcome to Terra Linda Athletics!  Joining a team is a great way for a student to make new friends and feel a part of the school community.  In order to participate in sports at Terra Linda:

  • The Athletic Registration must be completed online HERE.  
  • The CIF Pre-participation physical evaluation clearance form must be completed by your student’s physician and then uploaded onto FamilyID. 
  • Review all policies with your student (NCS/MCAL Ejection Policy, Student Athlete Contract, and Grounds for Suspension or Dismissal from a Team and conditions of participation)
  • If your student has transferred from another high school, please contact the Athletic Director regarding eligibility and completion of the appropriate NCS Eligibility Forms.

Expectations while playing sports at Terra Linda:

  • Participating in high school sports is a privilege not a right.
  • Students are making a commitment: to be on time, be prepared, be respectful, and be supportive. Terra Linda is your school – take pride in it.
  • Violations of school rules in either academics or athletics may result in the student’s removal from the team by the Athletics Department & their coaches (remember, sports are a privilege, and you are part of a team off and on the field).

Being Eligible to Play Sports:

  • As a member of the CIF (California Interscholastic Federation), students must maintain a 2.0 GPA and have no more than one failing grade.  This includes the last grading period of your 8th grade year.

Terra Linda High School has instituted a platform for athletic registration through FamilyID

FamilyID is a secure registration platform that provides you with an easy, user-friendly way to register for our programs, and helps us to be more administratively efficient and environmentally responsible.  When you register through FamilyID, the system keeps track of your information in your FamilyID profile.  You enter your information only once for each family member for multiple uses and multiple programs.


Athletes who participated in a sport  last year and have registered for FamilyID should do the following:

  1. Log into your account
  2. Click on Fall/ Winter/Spring Sports
  3. Click on the sport your student athlete wishes to participate
  4. Click on participant's name and all fields will populate with your information you already provided
  5. Scroll down the page until you find the agreements; click on all the agreements – Both Athlete and Parent/Guardian
  6. Edit any of your personal information if needed
  7. Press save


A parent/guardian should register by clicking HERE or go directly to FamilyID and follow these steps:

  1. To find your program, click on the link provided by the Organization and select the registration form under the word Programs.
  2. Next, click on the green Register Now button and scroll, if necessary, to the Create, Account Log In.  If this is your first time using FamilyID, click Create Account
  3. Create your secure FamilyID account by entering the account owner First and Last names(parent/guardian), E-Mail and password.  Select I Agree to the FamilyID Terms of Service.  Click Create Account.
  4. You will receive an email with a link to activate your new account.  If you don’t see the email, check your E-mail filters(spam, junk, etc…)
  5. Click on the link in your activation E-Mail, which will log you into
  6. Once in the registration form, complete the information requested.  All fields with a red* are required to have an answer.
  7. Click the Continue button when your form is complete
  8. Review your registration summary.
  9. No Payment Required
  10. Click the green Submit button.  After selecting Submit, the registration will be complete.  You will receive a completion email from FamilyID confirming your registration.

Once you are in the portal, under Links on the right hand side, click on the Pre-Participation Physical Form and print.  This is the only piece of paper you will need.  This is the form your doctor should sign and stamp and then you can upload the form to your account.  You can also find the Pre-Participation Physical Form by going to the Forms and Information tab above on the top left of the page.

If you have trouble uploading the physical form, the form may be brought in to the Athletic Office.

You may login at anytime to update your information and to check your registration(s).

To view a completed registration, select the Registration tab on the blue bar.
